A first call
If this were our first visit
We would stay in Lisbon. Walk a neighbourhood, take a tram for the hills, and go out to Belém if the hours are kind. Sintra is inland, and it is a different day. If the palaces are why you came, make Sintra the day rather than squeezing it on after lunch.
A first Lisbon call is better as a city you walk into than as a coach loop that tries to do Portugal before all-aboard.
A day ashore
Why Lisbon is worth getting off the ship for
Lisbon meets the water. The Tagus is in front of you, and the city climbs behind it in tiles and terracotta, neighbourhood by neighbourhood. From the ship it already looks like a place to walk into, not a stop to tick off.
On a cruise day the pleasure is those neighbourhoods: lanes in Alfama, the open squares of Baixa, a viewpoint over the river, a tram when the hills ask for it. Belém sits downriver with the tower, the monastery and a café counter worth joining if you have time. Food is not a sideshow here.
You can have a thoroughly good day without leaving town. Lisbon is usually docked, the centre is close from the usual berths, and the city still feels like Lisbon once you are a short walk from the gangway.
Choosing the day
What kind of Lisbon day suits you?
Lisbon tends to split into a city day or a Sintra day. Both are good. Doing both properly on one call usually is not.
See Lisbon properly
Neighbourhoods, a tram for the hills, and Belém if you have the hours. This is the day we would choose on a first visit, and it is realistic without a tour from the usual berths.
Give the day to Sintra
Wooded hills and palaces inland of the city. It needs a longer call. If this is the Portugal you came for, give it the whole day and leave Lisbon for another time.
Take it slowly
One neighbourhood, a long lunch, a viewpoint. Right when the call is short, or when you would rather sit in a café than chase Belém and the hills before all-aboard.
Private or small-group
When you want a guide, help with the hills, or Sintra handled for you. City tours and inland days are both set up around cruise hours, including mixed-age parties that would rather not piece together trams and taxis.

Maybe: not required in the city
From the usual Lisbon berths you can walk or ride into the centre and have a very good day without a booked excursion. Alfama, Baixa and a tram are realistic on your own if you leave enough time to be back for all-aboard.
Take a tour when you want Sintra (that journey is the tour), or when the hills, the heat or a short call make independent wandering unkind. A city tour also helps if you want Belém and the centre without piecing the trams together yourself.

Lisbon on a cruise day
Lisbon is a docked city call with hills, trams and an inland option that needs honest hours.
Where the ship docks
Lisbon is typically a docked port, not a tender. Ships use Santa Apolónia and Jardim do Tabaco by the old city, or Alcântara further west towards Belém. Confirm the berth the evening before; it changes how the morning starts. Alcântara sits nearer Belém; the other two sit closer to Baixa and Alfama.
Hills, trams and heat
Lisbon is hilly, and summer afternoons are tiring on foot. Use a tram between neighbourhoods rather than treating the whole centre as a walk. Tram 28 is slow and often full; it is a ride, not a plan for the day.
If you are thinking of Sintra
Sintra is inland. It is not a walk from the terminal, and it does not sit neatly on top of a city morning. Check the published hours on this page before you commit. A longer call suits Sintra; a shorter one suits staying in Lisbon.

Can I explore Lisbon independently?
Yes, from the usual berths. Santa Apolónia and Jardim do Tabaco sit close to Baixa and Alfama; trams and taxis cover the rest of a city day. Leave enough time to reboard, and do not treat Sintra as a late add-on.
City or Sintra?
A first Lisbon call is a city day unless Sintra is the reason you are in Portugal. If the palaces are the priority, make inland the whole day. Doing both properly on one call is what usually unravels the afternoon.
What could catch me out?
The hills, especially in summer; treating Tram 28 as the whole plan; not checking whether you are at Alcântara or by the old city; and trying to stack Lisbon and Sintra because both are famous.
